Each entry is written so your organization can adopt it as a local operational definition: what it tells you, an explicit numerator and denominator (or an explicit count), the unit, the period or cohort, the source categories, the accountable role, the required segmentation, the basis for any target, and the action to take when it moves the wrong way. Setting targets
- Start from your own organization baseline, measured over a stated period with a stated scope.
- Add matched peer comparison only where the peer figure has published inclusions and exclusions you can reproduce.
- Payer- and contract-specific deadlines are hard constraints, not targets to negotiate.
- Zero preventable missed filing deadlines is a legitimate aspiration and the only target here stated as an absolute.
- This toolkit does not invent numerical industry cutoffs. Where you see a number, it is either arithmetic or explicitly organization-adopted.

On the HFMA MAP framework
The HFMA MAP Keys provide a standardized way to define revenue cycle performance measures so that organizations describe the same thing when they use the same metric name. Their value to a leader is the discipline of the definition: a named numerator, a named denominator, a stated period, and stated inclusions and exclusions. That structure is what makes a number comparable over time and defensible in a finance review. Using a MAP figure as an external benchmark, however, requires more than the metric name — it requires the published inclusions and exclusions for that specific key, applied to data assembled the same way. HFMA's denial-metric guidance extends the same principle to denial terminology, where inconsistent local definitions of rejection, denial, and contractual adjustment routinely make denial rates incomparable between departments, let alone between organizations. This toolkit adopts that definitional discipline, not the benchmark values.
